An incorrect response of a protective relay—including unintended tripping, failure to trip, delayed or spurious operation—caused by errors in inputs, settings, logic, sensing equipment, firmware, communication, or environmental conditions such that the relay's action deviates from the intended protection scheme under defined fault or operating conditions.
