A breakdown in the process that ensures a patient understands, appreciates and voluntarily agrees to a proposed intervention, arising from inadequate disclosure of material information, failure to assess or support decision-making capacity, or the presence of coercion or undue influence, such that the patient's authorization is not legitimately informed.
