Provenance Loss

The loss or fragmentation of recorded provenance information—such as origin, custody events, timestamps, creators, and transfer or transformation records—that materially impairs the ability to verify an item’s origin, custody history, or chain-of-trust for purposes of authentication, attribution, legal compliance, or reproducibility.
