A physical penetration, tear, crack, or severe degradation of a power cable's outer sheath or jacket that exposes the underlying insulation, conductive elements, or fills to environmental agents (water, contaminants, oxygen) or electrical stress, thereby compromising dielectric integrity and mechanical protection.
